Here at PartnerUP Talent, we are SUPER excited to present this fantastic opportunity for an Occupational Therapist to join an incredible team in Hobart.
This is a newly created Care Partner position where you’ll combine your OT expertise with care coordination, client relationships and practical problem-solving to support older people to remain independent, safe and living well at home.
Let’s talk about the role
As part of an incredible team of Care Partners, you will manage a caseload of Support at Home clients, becoming a key person in their care journey.
- Build meaningful relationships with your clients and regularly visit them in their homes to assess their functional needs, independence and safety.
- Manage client budgets and ensure their supports meet their individual needs while remaining aligned with their available package.
- Work closely with your wider team as well as Nurses and Support Workers to achieve the best outcomes for your clients.
- Complete home modifications and safety assessments, helping clients remain safe and adapt their homes as their needs and circumstances change.
- Take a holistic approach to each client, looking beyond their clinical needs to understand what is important to them and how you can help.
Let’s talk about you
Yes, you are an Occupational Therapist with AHPRA registration — but you’re also passionate about why you do what you do.
Ideally, you’ll have experience in aged care, community, home care, rehabilitation or a similar setting, along with strong clinical reasoning, communication and problem-solving skills.
But if you’re a new graduate or your experience is from another clinical setting, don’t count yourself out. We’d still love to hear from you and talk about how we could support you in this role.
Most importantly, you’ll:

- Be confident working as part of a team and collaborating to achieve great outcomes.
