About the Role

As the Case Manager – Support at Home, you’ll walk alongside older adults receiving CHSP or Support at Home funding, helping them stay connected, confident and in control of their lives.

What You’ll Bring

• Certificate III/IV in Ageing, Disability, or Nursing (RN/EN highly desirable)
• Prior experience in a community, aged care or similar care-partnering role
• Strong understanding of restorative, enablement-focused care
• Ability to complete comprehensive assessments & develop meaningful, goal-driven care plans
• A natural connector — someone who brings out the best in clients, families, colleagues & partners
