We’re recruiting an experienced Occupational Therapist to deliver Support at Home assessments for clients across Sydney Metro and Tasmania. This is a community-based role focused on functional assessments, home modifications, and clear, high-quality reporting to support clients to live safely and independently at home.
This role can be based either in Sydney or Tasmania, with regular travel required across Sydney and interstate travel for assessments.
The Role:
You will work with older adults and people with disability, completing in-home assessments and providing practical, outcome-focused recommendations. You’re confident working independently in the community and enjoy assessment-focused work.
Key responsibilities include:
- Conducting Support at Home assessments for community clients
- Assessing functional capacity, safety, and environmental needs
- Providing home modification and equipment recommendations
- Completing clear, timely assessment reports and documentation
- Liaising with clients, families, and relevant stakeholders
- Managing your own schedule and travel to assessment locations
Essential requirements:
- Bachelor of Occupational Therapy (or equivalent)
- AHPRA registration
- Experience completing functional or community-based assessments
- Strong written skills, particularly for report writing
- Current Australian Driver’s Licence
- Willingness to travel across Sydney and interstate as required
Desirable:
- Experience in aged care, home care, or community OT
- Knowledge of home modifications and assistive technology
- Experience with government-funded programs (e.g. aged care or disability services)
