The Role:
- Deliver person-centred, enablement-focused support
- Conduct assessments and develop tailored care plans
- Monitor wellbeing and manage risks
- Coordinate services with clinical and allied health providers
- Manage allocated budgets in line with funding
- Build trusted relationships with participants and families
Every day is different — but every day is purposeful.
About You:
- Previous experience in aged or home care, ideally within case management
- Experience in healthcare administration is also welcome — training available for the right candidate
- Confident working independently and making sound decisions
- Current driver’s licence and National Police Check (or ability to obtain)
