In this office-based Community Care Partner role, you’ll provide crucial support to the local elderly community via telephone and email, helping clients remain safe, independent, and connected in their own homes.

The Role:
- Deliver person-centred, enablement-focused support via phone and email
- Monitor wellbeing and manage risks
- Coordinate services with clinical and allied health providers
- Manage allocated budgets in line with funding
- Build trusted relationships with participants and families
About You:
- Previous experience in aged or home care, ideally in case management
- Experience in healthcare administration is welcome — training provided
- Confident working independently and making sound decisions
- Current driver’s licence and National Police Check (or ability to obtain)
- Passionate about empowering older Australians and making a difference
